Why am I not seeing an actual call waiting notification?

If this just started happening, you can start by restarting your iPhone and then testing the issue. Restart your iPhone, iPad, or iPod touch
If your incoming callers are getting sent to voicemail instead of coming through as a call waiting, you can also make sure that the option for Call Waiting is turned on. Depending on your carrier, you may be able to configure this by going to Settings > Phone > Call Waiting. If you don't see the call waiting option, then you would want to contact your carrier to verify that you have call waiting set up.
